Fill out the application and either submit it by e-mail or postal mail. If you are using e-mail you must sign it with a digital signature. You will need Adobe Reader 7.0 or later to do this.
If you prefer to mail it in be sure to sign the document after filling it out - then drop it in the mail. In either case feel free to make an online payment. We can verify your address or name against the application to ensure proper payment. |
